National Health Fund: Children also in coordinated care. Facilities will receive an additional lump sum

Coordinators in primary care have been guaranteed funding since 2021, and this situation will remain unchanged. Starting October 1, 2025, remuneration will be calculated not only for adults but also for children registered with the clinic. Additionally, facilities that join the coordinated care system will receive a monthly incentive payment.
For four years now, the National Health Fund has been financing the coordinator's tasks through a capitation fee. Initially, this fee was payable for every patient over 24 years of age registered with a primary healthcare provider.
– “Tasks performed by the coordinator in primary health care facilities have been covered by financing under the capitation rate since 2021,” the National Health Fund reminds.
October 2022 brought significant changes – with the introduction of coordinated care, funding increased by 25%. Importantly, it covers patients aged 18 and over. From October 1, 2025, the directory will expand to include children, meaning the coordinator's fee will be calculated for all clinic patients.

This allowance is currently approximately PLN 7,700 for clinics with a population of up to 5,000 patients. It will be paid for six months from the date of entry into the system.
– “We are therefore maintaining the incentive nature of the coordinator lump sum. From the outset, we have clearly and openly stated that this is a fixed-term benefit,” emphasizes Paweł Florek , director of the National Health Fund’s Office of Social Communication and Promotion.
Coordinated care means more than just higher remuneration for the coordinator. The National Health Fund also funds a broader package of tests, specialist consultations, comprehensive family doctor consultations, as well as educational and dietary advice. This allows patients with chronic conditions to receive comprehensive treatment within primary care.
In 2024, the National Health Fund allocated over PLN 546 million for coordinated care. In the first quarter of 2025, this figure rose to PLN 148.2 million. Nearly 3,000 contracts for coordinated care have been signed to date, representing 48% of all primary healthcare contracts. Data show that 20.6 million patients, or approximately 60% of the population enrolled in primary healthcare, already use this form of care.
